JOB SUMMARY:As a Home Health Registered Nurse (RN) Case Manager, you will play a crucial role in assessing and managing the care of patients under Planned Parenthood California Central Coast Home Health (PPCCCHH). Your responsibilities will include conducting initial and ongoing assessments, developing and revising care plans in collaboration with patients and the Home Health Manager, and providing direct patient care.Conduct prenatal visits to assist patients in creating their postpartum care plans.Offer post-pregnancy care for patients following vaginal deliveries, cesarean sections, miscarriages, or abortions.Perform comprehensive assessments of patients’ physical, functional, psychosocial, and daily living activities, initiating preventative and rehabilitative nursing procedures.Enhance case management by ensuring quality home health care, accurate documentation, and effective referrals.Participate in training and orientation for new staff as needed.Coordinate home health care services effectively.Develop and update treatment plans in collaboration with patients and physicians as required.Provide direct care and education to patients and their families in alignment with established care plans.Secure and document physician orders for necessary services related to patients’ care plans.Manage patient pain levels through education on safe use of over-the-counter pain medications.Timely document patient interactions and responses in the electronic health record.
