Job Summary
We are looking for a friendly and compassionate Endocrinology Nurse Practitioner or Physician Assistant to join our dynamic team. In this pivotal role, you will provide comprehensive care to patients with endocrine disorders, including diabetes, thyroid conditions, and other hormone-related issues. Responsibilities
- Deliver comprehensive healthcare services and coordinate patient care
- Assess and treat patients with endocrinological diseases and conditions
- Collaborate closely with a team of healthcare professionals
- Evaluate, diagnose, and treat residents' physical and mental well-being
- Conduct thorough patient assessments, including medical history reviews and physical examinations
- Order and interpret diagnostic tests
- Formulate accurate diagnoses and treatment plans
- Prescribe medications and administer treatments
- Provide counseling to residents and their families
- Work with the interdisciplinary team for seamless care coordination
- Manage chronic conditions and monitor patient progress
- Make necessary adjustments to care plans
- Commit to person-centered care
- Create a safe and nurturing environment for elderly residents
Requirements
- Minimum 6 months of experience in long-term care or a skilled nursing facility
- Current licensure as a Nurse Practitioner or Physician Assistant in Massachusetts or Connecticut
- Strong communication and interpersonal skills
- Ability to work collaboratively in a team-oriented environment
