What you will do Assess hospice and palliative care patients of all ages to identify needs and provide music therapy interventions. Document patient issues, goals, timeframes, and interventions that are appropriate for each developmental stage. Deliver and record direct patient care according to the established Care Plan and department guidelines. Design and lead individual or group music sessions, adapting each session to the needs outlined in the Plan of Care. Monitor and document patient responses to therapy, updating records as needed.
