Our ambulatory youth team in the Delft area is currently seeking a dedicated Youth Support Worker to join us immediately. You will primarily work independently, providing assistance in the environments where it is needed, such as at school or at the client's home. A family coach oversees the entire care process and sets the objectives for the support. You will collaborate with the client on these goals and report on progress. Main responsibilities Mentor and support 10 youths aged 14 to 17 living at the Delft location. Many residents have autism spectrum disorders, ADHD, or similar needs. Follow individual care plans to assist residents in daily routines and encourage their personal growth. Help residents prepare for or transition into daytime activities, including volunteering, employment, or education. For those not yet ready for external activities, work together to set and achieve personal goals in manageable steps. Encourage, motivate, and provide hands-on support to help residents become more self-sufficient. Document daily progress and maintain regular communication with mentors. Participate in team meetings with colleagues, mentors, and behavioral specialists. Facility details The Delft residence offers 24-hour support in the city center and serves 30 clients, split between two groups: 20 adults and 10 youths. The main aim is to help these young people strengthen their skills and engage more fully in society. What you will do Guide and support youth as they develop social skills and work toward greater independence. Build professional, trusting relationships with each adolescent in your care. Collaborate with behavioral specialists, the youth themselves, and their parents or caregivers to set and review realistic goals. Monitor and document progress through regular evaluations. Engage each youth’s personal network, parents, caregivers, family, and friends, in the support process. Team and setting The team includes behavioral specialists, a location leader, mentors, guides, and hospitality staff.
