Project Worker, Greenhill's South Dublin Aftercare
Location: Dublin
Contract: Full Time, Specific Purpose Contract
As a Project Worker, you will play a vital role in the Aftercare Team, working on a 7-day rota to provide essential care and support to young individuals residing in the project as they transition towards independent living.
Key Responsibilities:
Engage with the overall aims and objectives of Focus Ireland, ensuring all practices uphold organizational values and standards.
Create and implement personalized care plans to equip and support each resident for independent living.
Offer comprehensive support to young people, covering practical self-care skills, health needs, budgeting, domestic skills, and confidence-building as they navigate their transition from care.
Advocate for residents regarding housing, social work, social welfare, education, training, and legal entitlements.
Collaborate with external agencies and services to address residents’ needs through appropriate referrals.
Provide individualized support and key-working sessions to residents to foster their ongoing development and progression.
Facilitate group work sessions for residents within the project.
Participate in a 7-day rota to meet the operational requirements of the service.
Help maintain high standards of hygiene and cleanliness within the project environment.
Stay alert to Health, Safety, and Welfare risks in the workplace, reporting any concerns to the Line Manager or Health & Safety Representative.
Keep accurate case records, statistics, and documentation, and inform the Project Leader of emerging issues in individual cases.
Engage in team meetings, supervision, training, project evaluations, and collaborative efforts with other Focus Ireland services, while taking on additional duties as assigned by management.
