These efforts may involve direct visits, phone calls, and collaboration with various agencies.Developing service plans addressing non-clinical needs (e.g., housing, transportation).Coordinating transportation to treatment sessions and community support appointments.Providing transportation to critical appointments when timely arrangements are not feasible.Advocating for clients to access local community support services.Offering assistance in obtaining urgent and standard Recovery Support services.Providing face-to-face supportive counseling.Establishing and maintaining connections with culturally relevant organizations and individuals to ensure adequate community support, including:Vocational providersHousing agencies and landlordsHealthcare providersProbation and parole officersCrisis intervention services and hospital emergency departmentsOther relevant support and treatment servicesFamily members and natural supportersInterpreter servicesFaith communities (e.g., churches, mosques)Guide clients through their treatment journey across different levels of care, including:Conducting assessments and referrals to appropriate care levels.Scheduling intake appointments.Reviewing treatment histories.Facilitating effective transitions between care levels to ensure continuity and quality of treatment.
