Prodeba is op zoek naar een gedreven ambulant begeleider voor de regio Hoofddorp. In deze rol ben je verantwoordelijk voor een eigen caseload van jongvolwassenen (18 tot circa 35 jaar) die te maken hebben met complexe problematiek, waaronder ontwikkelingsstoornissen zoals autisme, ADHD, hechtingsproblemen, somberheid en angstklachten. Lees dan snel verder!Wat houdt de functie van ambulant begeleider in?Als ambulant begeleider bied je ondersteuning aan jongeren en jongvolwassenen in hun dagelijkse leven. Je werkt zelfstandig en de begeleiding vindt plaats in de omgeving van de cliënt, zoals thuis. Je stelt samen met de cliënt een zorgplan op en bepaalt de doelen die je samen wilt bereiken. Tijdens teamoverleggen krijg je de kans om te overleggen over complexe hulpvragen en kun je, indien nodig, advies inwinnen bij een gedragsdeskundige.Wat wij jou biedenEen aantrekkelijk salaris tussen € 2.702,25 en € 3.826,48 bruto per maand op basis van een fulltime werkweek van 36 uur.
