Role overview
The Hospice Intake Nurse position is full-time, based in Oklahoma City, OK, and follows a hybrid work schedule. This role is central to the intake process for hospice care, coordinating closely with both patients and clinical staff. The team culture emphasizes quality care and mutual support among colleagues.
Key responsibilities
- Receive incoming calls and gather all referral and intake information.
- Complete, document, and route referral and intake forms for admission approval.
- Assist with scheduling skilled staff for patient cases and enter details into the scheduling system.
- Verify Medicare coverage and benefit periods using standard working files.
- Perform additional duties as assigned.
Work environment
- Office setting with frequent phone use, sitting, stooping, and occasional lifting of items like printer paper.
- Regular use of a PC, calculator, multi-line phone, and other typical office equipment.
Requirements
- High school diploma or GED.
- Multi-state LPN license preferred.
- Experience in Home Health Care or Hospice settings preferred.
- Basic computer skills and ability to maintain simple records.
- Strong oral and written communication skills.
- At least 3 years of customer service experience.
