Your Role:

As a dedicated and skilled Nurse Practitioner, you will play a vital role in delivering high-quality in-home wellness assessments for our Medicare members and other at-risk populations. Your primary goal is to evaluate the overall health and well-being of beneficiaries, ensuring accurate and comprehensive risk adjustment coding that supports enhanced value-based care. This PRN (1099) position offers exceptional flexibility, ideal for those managing multiple priorities.

 

Key Responsibilities:

In-Home Clinical Assessments:

  • Conduct thorough health evaluations in members’ homes.
  • Perform Annual Wellness Visits and assessments for individuals with chronic conditions.
  • Execute medical history and physical examinations, system reviews, vital signs collection, and medication reconciliations.
  • Administer preventive health screenings, including Hemoglobin A1C, spirometry, Kidney Health Evaluations (KED), Diabetic Retinal Exams (DRE), and FIT tests.

 

Patient Education & Engagement:

  • Deliver impactful health education and counseling.
  • Guide patients on health monitoring, medication adherence, and chronic disease management.
  • Facilitate informed decision-making and ensure genuine consent.
  • Engage in care goal discussions as appropriate.

 

Care Coordination & Quality Support:

  • Identify relevant clinical diagnoses for care management.
