Primary Purpose:

We take pride in the quality of care we extend to our members. As our team grows to meet the demands of our expanding business, we are in search of a highly skilled and compassionate Certified Nurse Practitioner to join our Advantmed provider network.

 

In this role, you will conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your primary goal will be to evaluate each member's overall health and well-being, ensuring accurate and comprehensive risk adjustment coding that enhances value-based care. This position is on a 1099 (PRN) basis, allowing for significant flexibility suited for professionals balancing various commitments.

 

NP Responsibilities:

  • Conduct in-home clinical assessments, including comprehensive health evaluations.
  • Perform Annual Wellness Visits and health assessments for members with chronic conditions.
  • Execute thorough medical history and physical examinations.
  • Review systems, collect vital signs, and conduct medication reconciliation.
  • Administer preventive health screenings, including Hemoglobin A1C, Spirometry, Kidney Health Evaluation (KED), Diabetic Retinal Exam (DRE), and FIT.

 

Patient Education & Engagement:

  • Provide preventive health education and counseling to members.
