Primary Purpose:

At Advantmed, we take pride in delivering exceptional care to our members. As we expand to meet increasing demands, we are seeking a dedicated and skilled Certified Nurse Practitioner to enhance our provider network.

 

In this role, you will conduct in-home wellness risk adjustment assessments for Medicare beneficiaries and other at-risk populations. Your primary goal will be to evaluate the overall health and well-being of members to ensure accurate and comprehensive risk adjustment coding, thereby facilitating superior value-based care. This position is classified as 1099 (PRN), offering significant flexibility for those balancing multiple responsibilities.

NP Responsibilities:

  • Perform in-home clinical assessments by traveling to members’ residences.
  • Conduct Annual Wellness Visits and health assessments for members with chronic conditions.
  • Execute comprehensive medical evaluations, including:
    • Medical history and physical examinations
    • Review of systems
    • Vital signs collection
    • Medication review and reconciliation
    • Preventive health screenings such as Hemoglobin A1C, Spirometry, Kidney Health Evaluation (KED), Diabetic Retinal Exam (DRE), and FIT
  • Deliver patient education and engagement through preventive health counseling.
