Role Overview

Advantmed is expanding its provider network in Multnomah County. The team seeks a certified Nurse Practitioner to perform in-home wellness risk adjustment assessments for Medicare beneficiaries and other high-risk groups. This is a 1099 (PRN) position, offering flexibility for those balancing multiple commitments.

What You Will Do

Conduct comprehensive health evaluations in members’ homes
Perform Annual Wellness Visits and assessments for those with chronic conditions
Assess medical history and complete physical examinations
Review systems and collect vital signs
Review and reconcile medications
Provide preventive health screenings, including:
- Hemoglobin A1C
- Spirometry
- Kidney Health Evaluations
- Diabetic Retinal Exams
- FIT (Fecal Immunochemical Test)

Offer educational support and resources to help members manage their health
