Role Overview
Advantmed is expanding its provider network in Multnomah County. The team seeks a certified Nurse Practitioner to perform in-home wellness risk adjustment assessments for Medicare beneficiaries and other high-risk groups. This is a 1099 (PRN) position, offering flexibility for those balancing multiple commitments.
What You Will Do
- Conduct comprehensive health evaluations in members’ homes
- Perform Annual Wellness Visits and assessments for those with chronic conditions
- Assess medical history and complete physical examinations
- Review systems and collect vital signs
- Review and reconcile medications
- Provide preventive health screenings, including:
- Hemoglobin A1C
- Spirometry
- Kidney Health Evaluations
- Diabetic Retinal Exams
- FIT (Fecal Immunochemical Test)
Patient Education & Engagement
- Offer educational support and resources to help members manage their health
