Position Overview
We are looking for a talented Clinical Case Manager (internally referred to as Clinical Lead Care Manager - CLCM) to join our innovative team. Reporting to the ECM Clinical Manager, the CLCM acts as the primary liaison for clients, collaborating with their healthcare providers—including doctors, specialists, pharmacists, and social services—to ensure coordinated care that meets the overall needs of the client. This role entails managing client cases, coordinating healthcare benefits, providing education, and facilitating timely access to care. The CLCM will also partner with clients' caregivers and other providers in the Care Team to enhance wellness, promote recovery and independence, and maximize member benefits.
This is a pivotal position that we aim to fill promptly.
Key Responsibilities
As a Clinical Case Manager, you will perform remote care management tasks, including:
Evaluate members' needs across various domains including physical health, mental health, substance use disorders, oral health, palliative care, memory care, trauma-informed care, social support, housing, and connection to community-based services.
Oversee the creation of personalized care plans and goal-setting processes.
Deliver services where clients are most comfortable, whether that be in their homes, through telehealth, or in the field.
Facilitate connections to essential social services and support systems.
Act as an advocate for clients when interacting with healthcare professionals.
Implement evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
Engage in outreach and engagement activities to enhance connections with the ECM program.
