Location: San Mateo County (Pacifica, Daly City, San Bruno, South SF).
Compensation: $21.25 per hour

Role Overview:
As a Community Support Facilitator, you will empower individuals to engage in activities and access services within community settings. You will tailor support based on the consumer’s Person Centered Plan and Individualized Personal Plan.
Key Responsibilities:
1. Provide tailored group and individual support in community environments as outlined in the consumer’s Individualized Personal Plan.
2. Create avenues for clients to be active, valued members of the community while fostering positive interactions with community members.
3. Develop and lead activities that enhance relationships, skills, and community engagement while accompanying clients throughout the local area.
4. Cultivate consumer participation in everyday community experiences, ensuring inclusivity and support.
