As a Home Care Assistant, you play a pivotal role in our team, offering essential support to our participants in their residences. You will also engage in the care planning process as an integral member of our interdisciplinary team (IDT). Reporting directly to the Home Care Supervisor, you will ensure the safe and efficient delivery of personal care services to WelbeHealth participants as per their scheduled needs.
What sets our Home Care Assistants apart at WelbeHealth:
- Enjoy a stable, full-time schedule; unlike traditional home care roles, you'll be assigned participants to ensure a full workday, with reassignments in case of cancellations or emergencies.
- Attend to 2-5 patients daily, catering to individual needs and care plans while working shorter shifts that accumulate to an 8-hour workday.
- Receive reimbursement for travel time and mileage while visiting participants.

Your daily responsibilities will include:
- Providing hands-on personal care and assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and safe physical lifting when transferring participants.
- Supporting participants with Instrumental ADLs, including laundry, light housekeeping, shopping, and meal preparation.
- Measuring and documenting participants’ vital signs as required.
- Acting as a medical escort to appointments, ensuring safe transfers in and out of their homes.
