Key Responsibilities:
- Perform home health visits to evaluate and monitor patients' health, including taking vital signs and reviewing medical histories.
- Administer prescribed medications and treatments as directed by physicians.
- Assist patients with daily living activities, such as bathing, dressing, and mobility, ensuring their independence.
- Collaborate closely with registered nurses and other healthcare professionals to create and implement personalized care plans.
- Provide education to patients and their families regarding health management and medication compliance.
- Accurately document patient care activities in electronic health records in a timely manner.
- Offer emotional support and encouragement to patients and their families.
- Engage in team meetings and contribute to initiatives focused on continuous quality improvement.
Qualifications:
- Current and valid LPN license in Arizona.
- Preferred minimum of 1 year nursing experience in a clinical or home health environment.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Self-motivated with effective time management abilities.
- Current CPR certification; additional certifications in home health or geriatrics are a plus.
- Access to reliable transportation and willingness to travel to various patient locations.
