Purpose:
We are in search of an experienced Nurse Practitioner or Physician Assistant to deliver exceptional primary and palliative care to homebound patients in the Gloucester County, NJ area. You will implement medical techniques and principles under physician supervision, catering to a diverse population with chronic health conditions.
What You'll Do:
- Deliver high-quality care to primary and palliative patients in and around Gloucester County, NJ.
- Conduct comprehensive health assessments through physical examinations, update medical histories, and devise optimal care plans.
- Communicate effectively with office personnel and clinical operations using various internal platforms.
- Provide compassionate, patient-centered care to a dedicated panel of patients.
- Participate in quarterly medical staff meetings.
- Formulate clinical impressions, diagnoses, and treatment strategies.
- Build trust and engage with patients, families, and caregivers during consultations.
- Order necessary diagnostic tests as warranted.
- Prescribe medications, supplies, and appropriate medical interventions as needed.
