You will play a crucial role within a multi-disciplinary healthcare team, providing skilled nursing and rehabilitative care, allowing clients to enjoy the comfort of their homes. How You’ll Make an Impact:
- Conduct diagnostic assessments including mobility tests, transfer status evaluations, strength and endurance measurements, balance assessments, and safety evaluations.
- Create and execute tailored care plans that encompass manual therapy exercises, gait training, and balance interventions.
- Utilize evidence-based practices to implement protective interventions aimed at preventing wounds and promoting healing.
- Continuously assess and modify the Physical Therapy care plan as needed.
- Provide education and guidance to clients, family members, and caregivers, ensuring they understand rehabilitative care and activities vital for promoting health, safety, and independent living.
- Document all observations, interventions, and evaluations related to patient care management using an advanced touch pad tablet.
