Key Responsibilities
- Deliver physical therapy services to patients in accordance with the physician's plan of care, including:
- Assessing and evaluating therapeutic and functional status, along with home environment considerations.
- Evaluating muscle strength, mobility, gait, and range of motion to identify rehabilitation potential.
- Directing and implementing physical therapy treatment plans.
- Educating patients and their families on the proper use and care of therapeutic devices.
- Identifying priority needs for physical therapy services.
- Reporting patient reactions to treatment and any changes in condition to the physician.
- Initiate physical therapy programs and provide instruction to other staff and family members regarding patient therapy goals during case conferences.
- Train patients in the use of prosthetic devices as needed.
- Assess needs for additional home health services and make referrals accordingly.
- Prepare and submit clinical progress summaries based on goal achievements.
- Participate in discharge planning for patients.
- Provide educational sessions for nursing personnel as required.
- Engage in peer consultation processes.
- Supervise physical therapy assistants in compliance with organizational policies and state regulations.
- In instances where therapy is the sole skilled service, instruct and evaluate home health aide care bi-weekly.
