Role Overview:
The Registered Nurse Case Manager is responsible for planning, organizing, and directing hospice care while leveraging their nursing expertise, particularly in community health education. This professional will collaborate with community resources to tailor services that address the unique needs of individuals and families in their homes and communities.
Key Responsibilities:
Patient Care
- Conduct comprehensive assessments of patients and families to identify hospice needs, including detailed physical evaluations and medical histories.
- Deliver professional nursing care by applying all elements of the nursing process.
- Assess and monitor patient status through:
- Developing and initiating care plans
- Regularly reviewing patient and caregiver needs
- Adapting care plans as necessary
- Formulating nursing diagnoses based on health assessment data.
- Creating care plans with established goals, incorporating palliative actions, and involving patients and families in the planning process.
- Implementing preventive and rehabilitative nursing procedures, administering prescribed medications, and treatments.
- Providing guidance and support to patients and families in addressing nursing and related needs.
