As a Social Worker reporting to the Social Work Manager, you will manage a caseload of participants while closely collaborating with the IDT. Your responsibilities will include conducting psychosocial assessments, developing care plans, providing counseling, and managing cases. You will also act as a vital link between participants, their families, support systems, and care teams to facilitate optimal care.

Your daily responsibilities will include:
- Conducting thorough assessments to identify the psychosocial needs, preferences, and goals of participants, while actively participating in IDT meetings to create personalized care plans.
- Implementing and documenting social work interventions in line with care plans, which may involve resource coordination, care transition assistance, individual and group counseling, and case management.
- Collaborating with Primary Care Physicians and IDT members to ensure seamless transitions of care across various settings (e.g., hospitals, skilled nursing facilities, home).
- Coordinating and facilitating care conference meetings to enhance care coordination among team members, participants, and their support networks.
- Providing discharge planning and follow-up care to ensure continuity and effectiveness of care.
