- During an initial consultation, review the patient's medical history, previously received information, and daily habits.
- Collaborate with the patient to create a personalized follow-up plan tailored to their needs. Topics include:
- Learning to use self-monitoring equipment
- Explaining the disease and treatments
- Providing lifestyle advice: nutrition, physical activity, smoking cessation, etc.
- Offering practical tips for daily life: diabetes and travel, driving licenses, etc.
- Being attentive to the patient
- Scheduling educational sessions where the diabetes educator will provide support, especially during treatment changes.
- In partnership with the patient, set achievable goals and monitor their progress (e.g., weight loss, smoking cessation, frequency of blood sugar checks, etc.).
- Collaborate on the health monitoring of diabetic patients:
- Review patients' self-monitoring logs, collect objective parameters, and analyze them (e.g., instances of hyper- or hypoglycemia).
- Gather data from the patient's diabetes assessment, observe their general health status (e.g., weight changes, blood pressure, lab results, foot examinations), and identify any potential changes.
- Adjust patient treatment as necessary in consultation with or under a physician's prescription (e.g., adjusting the type of insulin administered).
- Ensure follow-up on prescribed health assessments or medical exams.
- Contribute to the continuity and quality of care:
- Inform the physician about patients' health status and consult with them regarding recommended treatments.
- Share patient information with various healthcare providers (e.g., podiatrists, psychologists).
- Participate in multidisciplinary meetings and implement agreed-upon decisions.
- Contact external organizations to ensure patient follow-up.
- Perform administrative tasks related to patients:
- Create and maintain patient files (e.g., completed exams, medical supplies provided).
- Handle various formalities related to diabetes agreements or care pathways and ensure follow-up.
- Manage medical supplies for patients:
- Oversee inventory of materials (e.g., pumps, glucometers, informational brochures) and medications (e.g., insulin): check stocks and place orders according to internal procedures.
- Select materials based on patient needs and provide them according to diabetes agreement terms.
- Receive suppliers, consult with them, and ensure proper supply management.
