Primary Purpose:
We take pride in delivering exceptional care to our members. As our organization rapidly expands to meet the demands of our growing business, we are in search of a highly skilled and compassionate Certified Nurse Practitioner to join the Advantmed provider network.
In this role, you will conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your main goal will be to evaluate the overall health and well-being of our member beneficiaries, ensuring accurate and comprehensive risk adjustment coding that enhances value-based care. This position is classified as 1099 (PRN) offering remarkable flexibility for individuals balancing various commitments.
NP Responsibilities:
In-home Clinical Assessments:
- Travel to members’ residences to perform comprehensive health evaluations.
- Conduct Annual Wellness Visits and health assessments for members with chronic conditions.
- Perform medical history and physical examinations, review systems, collect vital signs, and conduct medication reviews and reconciliations.
- Facilitate preventive health screenings such as Hemoglobin A1C, Spirometry, Kidney Health Evaluations, Diabetic Retinal Exams, and FIT.
Patient Education & Engagement:
- Provide preventive health education and counseling to enhance patient engagement.
