Key Responsibilities:
Patient/Family and Community Relationship
Care Coordination
- Conduct thorough assessments for all assigned patients, ensuring accurate documentation.
- Promptly communicate any changes in patient conditions.
- Utilize nursing diagnoses and assessment data to establish realistic, measurable goals within the care plan, modifying it as necessary.
- Identify and prioritize patient needs, documenting appropriately.
- Collaborate with patients, families, and healthcare team members during care planning while ensuring proper documentation.
- Evaluate patient responses to nursing interventions and adjust care plans as needed.
- Assess patients for discharge planning and make necessary referrals, including palliative care.
- Ensure comprehensive discharge instructions, medication reconciliation, and provide information on patient portal usage and other resources to patients and families.
Patient Education, Ethics, and Diversity
- Support the spiritual and cultural needs of patients and families during assessments, documenting education levels and readiness for learning.
- Implement the teach-back method to enhance patient education regarding medications and side effects.
- Provide educational resources like Krames for clinical conditions.
- Incorporate cultural awareness into care planning and delivery.
- Treat all patients, families, and staff with respect and dignity.
- Value the contributions of significant others as essential to the care plan.
- Adhere to ethical standards and seek ethics consultations as needed.
Evidence-Based Practice and Research
- Implement nursing interventions that align with established evidence-based care plans.
- Continually advance the professional image and practice of nursing.
