About the role

The Registered Nurse (BSN) exemplifies the principles of the Relationship-Based Care model while executing core nursing functions at Northwestern Medicine. The position requires adherence to the Illinois Nurse Practice Act, the organization’s Code of Ethics, Corporate Compliance Program, and compliance with all relevant policies, procedures, guidelines, and regulatory standards.

Key Responsibilities:

Patient/Family and Community Relationships

Care Coordination

  • Conduct comprehensive assessments for all assigned patients, ensuring accurate documentation.
  • Promptly communicate any changes in patient conditions.
  • Develop realistic outcomes based on nursing diagnoses and assessment data, documenting measurable goals in the care plan; adjust the care plan as necessary to reflect realistic patient goals.
  • Identify and prioritize patient needs and document nursing diagnoses.
  • Collaborate with patients, families, and the healthcare team in care planning, including assessing discharge needs and making necessary referrals.
  • Evaluate and document patient responses to nursing interventions, modifying care plans as required.
  • Ensure all discharge instructions are completed, medication reconciliations performed, and provide essential information on patient portals and resources.

Patient Education, Ethics, and Diversity

  • Address patient and family spiritual and cultural needs during assessments and educational interactions; document the patient's readiness and learning level.
  • Utilize teach-back methods to enhance patient education regarding medications and their side effects.
  • Distribute educational resources, including Krames materials for clinical conditions.
  • Incorporate cultural considerations in care planning and delivery.
  • Respect and dignify all patients, families, and staff interactions.
  • Recognize the importance of significant others in the patient's care plan.
  • Adhere to ethical practice standards and consult ethics committees when necessary.

Evidence-Based Practice and Research

  • Implement nursing interventions that align with established evidence-based care plans.
  • Promote the professional image of nursing and the science of professional nursing practice.
