Job Title: Healthcare Coordinator
Location: 100% Remote (US Only)
Compensation: $45,000-$55,000 + Full Benefits Package
As a Healthcare Coordinator at Served With Honor, you will be instrumental in enhancing our innovative care delivery model. This pivotal position aids veterans in obtaining medical evidence for VA disability claims, while also laying the groundwork for expanded clinical services that may cater to veteran family members and caregivers. You will ensure the prompt and accurate completion of intake and documentation processes, which are essential for delivering high-quality care across all service lines. As we progress in refining our integrated care model, your role will be crucial in facilitating multi-disciplinary coordination and clinical operations.
Immediate Responsibilities:
- Coordinate care preparation activities, including intake form readiness and documentation tracking.
- Engage with veterans to collect, verify, and clarify necessary information prior to service delivery.
- Assist with basic billing-related tasks, such as confirming payment statuses, collecting payments, and identifying payment issues until full customer service transition is complete.
- Organize and upload medical records and supporting documents into clinical systems.
- Confirm appointment logistics and ensure veterans are prepared for scheduled services.
- Monitor CRM/EHR task queues to ensure timely resolution of assigned workflows.
- Act as the primary contact for veterans during the intake-to-service process.
Long-Term Responsibilities:
- Contribute to the development of streamlined workflows for care coordination.
- Support initiatives aimed at expanding services beyond medical evidence to enhance care delivery for veterans and their families.
- Collaborate with Quality Assurance, Provider Network Management, and clinical leadership to improve service continuity and provider readiness.
- Engage in data-informed process improvements and clinical readiness initiatives.
- Serve as a resource for future clinical integration efforts, including support for RN-led and multidisciplinary teams.
