As a Home Care Assistant, you will play a crucial role in our interdisciplinary team (IDT), supporting our participants in their homes and actively participating in the care planning process. Under the supervision of the Home Care Supervisor, you will ensure safe and effective delivery of personal care services as scheduled.

- You will have a stable, full-time schedule, with designated participants to fill your workday, and reassignments only in cases of cancellations or emergencies.
- Depending on participant needs, you will visit 2-5 patients per day, allowing for shorter individual shifts that still total an 8-hour workday.

Your daily responsibilities will include:
- Providing hands-on personal care and assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and transferring participants safely.
- Assisting with Instrumental ADLs, including laundry, light housekeeping, shopping, and meal preparation.
- Measuring and documenting participants’ vital signs as instructed.
- Acting as a medical escort for participants during appointments, ensuring safe transfers to and from vehicles.
- Supporting the overall care process as a valued member of the WelbeHealth team.
