Work Schedule: Sunday to Tuesday from 8:00 AM to 7:00 PM; Wednesday from 12:00 PM to 5:00 PM (70 hours bi-weekly)

Primary Responsibilities:
The In Reach Worker will focus on supporting tenants dealing with hoarding issues and maintaining cleanliness in their living spaces. This role includes assisting residents in developing life skills and participating in enriching activities as outlined in their individual support plans.
The In Reach Worker is responsible for providing essential programs and services to help tenants thrive within Pacifica Housing, which includes:
- Offering individualized, person-centered support services aligned with a “housing first” approach;
- Establishing meaningful connections with tenants;
- Assisting individuals with hoarding behaviors;
- Supporting tenants in daily living activities;
- Responding to overdose situations and administering naloxone.
Key Duties Include:
- Delivering tailored, person-centered support to residents, which involves:
- Preparing, documenting, implementing, monitoring, and evaluating personalized support plans;
- Collaborating to execute a person-centered Wellness Model;
- Providing insights to counselors and other professionals to develop appropriate program plans;
- Monitoring tenants' mental health and substance use, and identifying when additional support is needed;
- Recognizing and responding to potential emergency situations;
- Encouraging tenant participation in decision-making regarding their housing and support.
