About the Role
Internal Title: Clinical Lead Care Manager
We are on the lookout for an outstanding Clinical Lead Care Manager (CLCM) to join our dedicated team. Reporting to the ECM Clinical Manager, the CLCM acts as the primary point of contact for clients, liaising with medical professionals, specialists, pharmacists, and social service providers to ensure a unified approach to client care. The CLCM is responsible for managing client cases, coordinating healthcare benefits, providing education, and facilitating access to timely and cost-effective care. This role requires collaboration with caregivers, family support persons, and the Care Team to foster wellness, recovery, independence, and empowerment, ensuring access to necessary services and maximizing member benefits.
This is a hybrid position requiring travel across the Los Angeles County area up to five days per week. Candidates must reside within 20 miles of the assigned territory due to the frequency of travel.
This position is critical, and we are eager to fill it as soon as possible.
Key Responsibilities
As a Clinical Lead Care Manager, you will perform hybrid (in-person and remote) care management duties, including:
Evaluating member needs across various domains: physical health, mental health, substance use disorders, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and linking to community-based services.
Guiding the development of personalized client care plans and goal setting.
Providing services in locations most accessible to members, including office, telehealth, or field settings.
Connecting clients to necessary social services and resources.
Advocating for clients to ensure they receive the support and resources they need.
