Position Overview:
As a Nurse Practitioner, you will assess, diagnose, order and interpret medical tests, prescribe medications, and collaborate closely with our multidisciplinary health care teams to provide exceptional care to our Residents.
Key Responsibilities:
- Conduct comprehensive assessments, diagnoses, treatment plans, health education, and counseling in accordance with the standards set by the College of Nurses of Ontario (CNO).
- Contribute to accurate chart documentation and engage in chart reviews and case conferences.
- Ensure continuity of care by collaborating with primary care providers and health care teams.
- Deliver primary or shared medical care, consultations, and follow-ups to Residents and their families.
- Perform advanced assessments, develop diagnoses, and create therapeutic care plans, including the prescription of medications.
- Effectively communicate findings and diagnoses to Residents, families, and the care team, discussing health outcomes.
- Assume a leadership role within the home, fostering innovative solutions that enhance the quality of care for our Residents.
- Support staff in delivering exemplary care while adhering to their full professional scope.
- Encourage and exemplify critical thinking to identify needs and recommend improvements for best practice outcomes.
- Consult with physicians in accordance with established standards for physician consultation.
- Maintain professional relationships with networks, related organizations, and service providers.
- Ensure accurate, concise, and confidential documentation of Resident history.
- Prepare weekly and monthly reports as necessary.
- Advocate for Resident Rights.
- Perform other duties as assigned by the supervisor.
