Your Role
As a Nurse Practitioner, you will engage with members in your designated area to perform in-home health risk assessments. This will include evaluating medical histories, conducting comprehensive physical examinations, reconciling medications, and measuring vital signs. Your assessments will utilize evidence-based practices and incorporate point-of-care testing as needed, while identifying diagnoses for follow-up care management. You will play a crucial role in developing a detailed list of medical conditions based on your clinical insights and patient evaluations.
Qualifications
- Active and valid Nurse Practitioner license in Ohio
- Minimum of 3 years of patient care experience, with a preference for backgrounds in primary care, adult/geriatric care, and familiarity with electronic medical records (EMR).
- A strong commitment to empathy, compassion, and interpersonal friendliness.
- Exceptional verbal and written communication skills, exhibiting sincerity and respect when assisting patients and colleagues.
