About the Role:
The On-Call Nurse plays a vital role in addressing the patient needs of our agency after hours. This professional nurse utilizes community resources to plan and direct services tailored to the needs of individuals and families within their homes and communities outside of normal business hours.
Key Responsibilities:
- Conduct initial and comprehensive assessments of patients and families to identify hospice needs, including a thorough physical assessment and medical history review.
- Deliver professional nursing care by leveraging all elements of the nursing process, applying health assessment data to establish nursing diagnoses.
- Evaluate patient status by drafting and implementing care plans, reassessing patient and family/caregiver needs after hours, and participating in updating care plans as necessary.
- Formulate care plans that set achievable goals based on nursing diagnoses, incorporating palliative nursing actions and involving patients and families in the planning process.
- Initiate appropriate preventative and rehabilitative nursing procedures, administering medications and treatments as prescribed by the physician within the care plan.
- Assist patients with daily living activities, supporting their journey toward self-sufficiency and optimal comfort care.
- Collaborate with the clinical team to ensure comprehensive care.
