Job Purpose
The Ambulatory Clinical Pharmacist plays a pivotal role in enhancing the quality of pharmacotherapy care for oncology patients. Working in an outpatient oncology setting, this position emphasizes collaboration with healthcare team members through a practice-based care model. The pharmacist engages in direct disease management with patients, both face-to-face and via phone consultations. Responsibilities include reviewing prior authorization requests for medications, ensuring seamless communication among healthcare professionals, and focusing on patient outcomes related to pharmacotherapy. The pharmacist will identify, prevent, and resolve medication-related issues, assessing each patient's medication regimen for effectiveness, safety, and adherence.
Responsibilities
- Manage medication therapy by reviewing and monitoring medication regimens, advising clinicians on evidence-based therapies, and documenting interventions.
- Evaluate clinical chart information to determine the appropriateness of requested treatments.
- Oversee and review prior authorization requests for medications.
- Develop clinical criteria and medical policies for medication review.
- Ensure compliance with regulations set forth by the Center of Medicare and Medicaid Services (CMS).
- Maintain compliance with FDA guidelines and relevant clinical literature.
- Organize, manage, and update pertinent data using database applications.
- Provide operational insights and recommendations to enhance productivity and service quality.
- Identify trends and implement improvements.
- Actively participate in process improvement initiatives.
- Safeguard organizational information by maintaining confidentiality.
- Enhance professional knowledge through educational opportunities and participation in professional organizations.
