Position: Part-Time Physical Therapist Case Manager
Coverage Area: Fremont/Hayward Area
As a Physical Therapist Case Manager, you will collaborate with other clinicians to provide skilled services that evaluate and meet the needs of individuals in their homes, enabling them to live safely and productively. Your primary goal will be to restore function, prevent or minimize disability following illness or injury, and assist clients in achieving their maximum performance within their capabilities. You will evaluate and treat patients based on medically prescribed physical therapy programs and may supervise Physical Therapy Assistants (PTAs) in accordance with agency and regulatory standards.
