ORGANIZATIONAL RELATIONSHIPS
Reports to the Director of Rehabilitation (DOR) and Supervising Staff Physical Therapist (PT).
RESPONSIBILITIES
Administer patient treatments as prescribed by the Physical Therapist.
Document all treatments provided in the patient's medical records.
Engage in Daily and Weekly Clinical Case Management Meetings as required.
Coordinate with the supervising therapist and interdisciplinary team members regarding patient progress and care plans.
Contribute to in-service training programs at the facility.
Maintain accurate records of daily treatments and labor as per corporate policies.
Guide families and staff on maintenance programs as directed by the Physical Therapist.
Assist in the upkeep and cleanliness of the treatment area and department.
Ensure daily Treatment Encounter Notes are recorded before end of business hours.
Complete Daily Encounter Record billing prior to closing business.
Submit Weekly Progress notes every 7 calendar days or every 5 treatment days.
QUALIFICATIONS
Physical and Sensory Requirements (with or without mechanical assistance):
Mobility (including walking, balance), reaching, bending, lifting, grasping, fine motor skills, visual and auditory ability, literacy, and effective communication with patients, families, and staff, while maintaining composure under stress.
Graduate from an accredited Physical Therapy Assistant program.
Current licensure as a Physical Therapy Assistant in the practicing state.
Proficient in English communication, both spoken and written.
Strong interpersonal skills and the ability to relate effectively to patients and team members.
