Join our dedicated team at Mercy LIFE Assisi House, where you will play a vital role in providing high-quality primary medical care to participants of the PACE Organization (PO). As a Primary Care Physician (PCP), you will work under the supervision of the medical director and collaborate with an interdisciplinary team to assess patient needs, develop tailored care plans, and deliver exceptional medical services.
Your responsibilities will include conducting initial and ongoing assessments, managing participant care in our multi-level care facility, and ensuring effective communication with nursing home staff to monitor patient progress. You will also have the opportunity to refer participants to specialists as needed and contribute to continuous improvement by reviewing medical outcomes.
This position is ideal for those who thrive in a collaborative environment and are eager to make a difference in the lives of our senior participants, including those in specialized units for Alzheimer's and dementia care. Candidates should be open to learning American Sign Language to better serve our community of deaf seniors.
Work schedule includes two 8-hour days at Assisi House and one 4-hour day at Valley View, with flexible weekday scheduling and no requirements for night, weekend, or holiday coverage.
