The Registered Nurse Case Manager plays a pivotal role in guiding participant care as an integral member of the interdisciplinary team (IDT). Under the supervision of clinical leadership, the Case Manager oversees all aspects of participant care, which includes performing essential PACE assessments, crafting and implementing personalized care plans, and facilitating effective transitions of care.

In your daily activities, you will:
- Conduct nursing assessments to identify participant needs, preferences, and goals, and actively engage in IDT meetings to create participant care plans.
- Implement and document nursing interventions as outlined in participants’ care plans, ensuring a safe environment, responding promptly to physician orders, and administering medications correctly.
- Collaborate with the WelbeHealth PACE Physician and other IDT members to coordinate participant care, while also communicating with participants and their families to adjust care plans as needed.
- Educate participants, their families, and caregivers on disease processes, self-care techniques, and preventative strategies.
