Key Responsibilities:
- Oversee patient census with a focus on resolving gaps in their clinical and non-clinical care.
- Identify and coordinate community resources that can enhance patient care.
- Provide education on chronic condition management to improve health literacy for patients.
- Manage medication protocols, including identifying issues, reconciliation, adherence, and refill coordination.
- Ensure prompt delivery of necessary services including Home Health, DME, Home Infusion, and more.
- Encourage communication with patients to address their questions and needs effectively.
- Facilitate referrals and appointment scheduling as necessary.
Essential Qualifications:
- Valid Compact/Multi-State license (LPN or LVN)
- Proficient in Microsoft Office Suite and skilled in communication.
- Strong written and verbal communication abilities.
Preferred Qualifications:
- Experience with Epic systems
- Bilingual capabilities
- Additional state licenses
Remote Work Requirements:

- A dedicated home office or a compliant workspace that meets HIPAA standards.
Physical Requirements:
- This role is primarily sedentary and will require prolonged periods of sitting.
- Ability to communicate clearly and listen attentively via phone for extended durations.
- Capability to comprehend, process, and take detailed notes during phone conversations in real-time.
