Your Role
As a Community Health Worker (CHW) at Evergreen Nephrology, you will play a crucial role in delivering high-quality, patient-centered care. This includes exceptional patient service, care coordination, and connecting patients to community-based resources. You will act as a trusted link between diverse, underserved populations and the health and social service systems, with an emphasis on building trust, engagement, and empowering patients.
Collaborating with an interdisciplinary team of Nurse Practitioners, Nurse Care Managers, Care Coordinators, Dietitians, Pharmacists, Licensed Clinical Social Workers, Population Health Social Workers, and Psychiatrists, you will utilize Evergreen’s integrated technology platform to help patients access services tailored to their individual needs. Moreover, you will partner with clinical teams and leadership to support community outreach initiatives aimed at promoting, maintaining, and enhancing the health and well-being of patients and their families.
In this role, you will lead comprehensive care coordination efforts, focusing on identifying and addressing Social Determinants of Health while working closely with the interdisciplinary care team. Your expertise will be invaluable to patients as you assist them in navigating the healthcare landscape, ensuring they receive the care and support they deserve.
