Qualifications
Key Responsibilities:Oversee patient census with a solutions-oriented approach to bridge gaps in patient care, both clinical and non-clinical.Identify and coordinate community resources that can enhance patient care.Deliver educational support and enhance health literacy concerning chronic condition management.Manage medication protocols, including identifying potential issues, reconciliation, adherence, and refill coordination.Ensure timely delivery of critical services such as Home Health, DME, and Home Infusion.Address patient inquiries and foster open communication to better understand their needs.Assist in managing referrals and organizing appointment schedules.Essential Qualifications:Current Multi-State/Compact License (LPN/LVN)Proficient in Microsoft Office SuiteStrong written and verbal communication skillsPreferred Qualifications:Experience with Epic SystemsBilingual abilitiesAdditional state licensures (LPN)Remote Work Requirements:Must have active high-speed Wi-FiHome office setup compliant with HIPAA regulationsPhysical Requirements:This is a sedentary role requiring prolonged periods of sitting.Ability to communicate clearly and listen attentively, often over the phone, for extended periods.Capability to comprehend, process, and take thorough notes in real-time during phone conversations.
