Qualifications
Key Responsibilities:Oversee patient census with a focus on resolving clinical and non-clinical care gaps.Identify and connect patients with community resources that enhance their healthcare experiences.Educate patients on chronic condition management and promote health literacy.Manage medication adherence, reconcile medications, and coordinate refills.Ensure timely service delivery for home health, DME, home infusion, and other essential needs.Foster open communication to address patient inquiries and needs.Assist with referrals and appointment scheduling.Requirements:Current Multi-State/Compact LPN License (NLC/LVN)Proficiency in Microsoft Office SuiteStrong written and verbal communication skillsPreferred Qualifications:Experience with Epic systemsBilingual candidates are encouraged to applyAdditional single-state LPN licensesRemote Work Requirements:Reliable high-speed internet connectionDesignated home office or HIPAA-compliant workspacePhysical Requirements:This role is primarily sedentary, requiring prolonged periods of sitting.Ability to communicate clearly and listen attentively over the phone for extended durations.Capable of taking detailed notes during phone conversations in real time.
