Your Role
As a Licensed Practical Nurse (LPN) at Evergreen Nephrology, your responsibilities will include conducting monthly clinical check-ins, identifying and addressing clinical concerns, reinforcing care plans, escalating issues appropriately, and ensuring the accuracy of clinical documentation across both the EHR and partner technology platforms. This position necessitates clinical judgment, professional expertise, and the ability to support patients between office visits while collaborating closely with Evergreen’s providers, care teams, and leadership. LPNs play a crucial role in delivering a safe, compliant, and clinically credible Connected Care Program.
Our program prioritizes SMS as the primary channel for patient engagement, with phone outreach utilized when necessary.
While this position is fully remote, candidates must be available to work from 8:30 AM to 5 PM in the Pacific Time zone.
Key Responsibilities
- Patient Engagement & Outreach
- Conduct monthly CCM touchpoints for assigned patient panels.
- Establish strong, ongoing relationships with patients to enhance care delivery.
