As a Healthguide, you will collaborate with clinical teams to enhance the connection between patients and their primary care physicians, ultimately improving health and well-being. The Healthguide plays a crucial role in patient engagement, helping them navigate their healthcare journey, resolve issues, and communicate effectively with their medical practices.
Key Responsibilities:
- Primarily working remotely, you will coordinate with the patient's medical practice staff and Guidehealth’s Case Management team to manage a specific patient population.
- Functioning autonomously while collaborating closely with medical practices.
- Assisting patients to achieve improved health outcomes by addressing their inquiries and concerns, connecting them with medical practices, implementing impactful care plans, and closing care gaps. This includes escalating medical issues and guiding patients to appropriate resources such as specialty care and community-based organizations (CBOs).
- Meticulously documenting patients’ medical histories and issues within the electronic health record (EHR).
- Understanding the referral and prior authorization processes.
- Building and maintaining trusting, professional relationships with assigned medical practices and within the Guidehealth organization.
- Meeting assigned Key Performance Indicators to ensure positive patient health outcomes while reducing medical costs.
- Utilizing effective communication methods (calls, texts, portals, emails) to engage and schedule high-risk patients.
- Appropriately escalating medical concerns to the Health Guide Registered Nurse, Primary Care Physician, or Guidehealth leadership.
- Adhering to all security policies and procedures to protect all PHI (Protected Health Information) under Guidehealth’s care as well as Guidehealth Intellectual Properties.
- This position may require compensated travel (10-15%) to clinical sites in the Atlanta, GA area for patient and provider engagement, including conducting retinal eye exams, with paid training provided.
