About the Role
As an Intake Consultations Nurse Practitioner at Baba, you will conduct concise telehealth consultations that establish medical necessity for our care navigation services. These consultations are pivotal in identifying high-risk health conditions and unmet social needs while confirming eligibility for ongoing support.
You will collaborate closely with our care advocates and clinical operations team to ensure that patients are enrolled in the appropriate level of care with safety, compliance, and compassion. This role is a remote, 1099 (contractor) position, ideal for healthcare professionals seeking flexible, impactful telehealth opportunities.
What You’ll Do
- Conduct initiating telehealth consultations, performing E/M or Annual Wellness Visits (AWVs) to establish medical necessity and identify high-risk conditions.
- Document findings accurately in Baba’s platform, ensuring proper coding and utilization of SDOH Z-codes, diagnoses, and risk factors.
- Validate and develop individualized care plans, ensuring alignment with the patient’s treatment needs and ongoing documentation of medical necessity.
- Work collaboratively with advocates and care managers to finalize personalized care plans addressing each patient’s medical and social needs.
