As a leading provider in the Treptow-Köpenick district, our organization offers a specialized residence for individuals facing challenging life situations. We provide a stable living environment for up to 150 residents, assisting them in navigating daily life, strengthening their resources, and coordinating necessary support services.
Our residents have complex social and health support needs, many of whom live with chronic illnesses, substance dependencies, or multiple stressors. Our interdisciplinary social service team collaborates closely with an outpatient nursing service and other medical partners to ensure a stable living situation and reliable care.
To enhance our social services team, we are looking for a dedicated
Social Worker / Social Educator (m/f/d)
who is passionate about professionally supporting individuals with complex social and health needs.
Your Responsibilities
- Independent case management for clients, including intake, assessment, need evaluation, and application for relevant benefits (e.g., SGB II, V, IX, XI, XII)
- Support and guidance in managing daily practical issues while promoting individual resources
- Coordination of support services and networking with external organizations such as care providers, therapists, doctors, hospitals, and specialized agencies
- Conducting counseling sessions with a life-world-oriented approach
- Participation in interdisciplinary case work, team meetings, and professional discussions
- Administrative and organizational tasks
- Planning and implementing additional activities for residents
