Job Purpose:
The Specialty Pharmacy Liaison plays a crucial role in delivering comprehensive off-site support to patients, pharmacists, and healthcare providers, ensuring a seamless approach to exceptional patient care. This role requires close collaboration with patients to provide a holistic range of medication adherence assistance through various tools and applications. The Liaison will actively encourage patients affiliated with specialty clinics to take full advantage of the hospital partner pharmacy's superior patient care offerings.
Job Responsibilities:
- Deliver outstanding care to hospital patients through a customized suite of medication management services.
- Act as an integral off-site member of the clinic team, facilitating prior authorizations, financial assistance, patient onboarding, and refill support.
- Educate potential pharmacy patients on the benefits of utilizing the hospital pharmacy for optimal care.
- Address all medication-related needs of patients.
- Expand the patient base for specialty pharmacies by enthusiastically adhering to Shields’ programs and processes.
- Handle insurance issues, including benefit investigations and prior authorizations on behalf of providers.
- Assess patients' financial assistance needs and secure support where applicable.
- Maintain accurate documentation within the hospital EMR and Shields' proprietary tools.
- Ensure patients adhere to their medication regimens through follow-ups and refill reminders.
- Facilitate prescription renewals.
- Communicate with patients to enhance their understanding of their medications.
- Identify medications necessitating special handling or storage conditions.
- Determine the necessary supplies for specific medications.
- Manage inbound inquiries with professionalism and efficiency.
- Contribute to operational targets by entering data, adjudicating test claims, and preparing follow-up plans.
- Implement suggestions for improving operations, productivity, quality, and customer service.
- Engage in initiatives aimed at process enhancement.
- Safeguard the organization’s reputation by maintaining confidentiality.
