BAYADA Home Health Care is actively searching for a skilled Speech Language Pathologist (SLP) to join our team on a per diem basis, providing home health visits. This position involves servicing adult and geriatric patients in the comfort of their homes across regions including Bath, Wind Gap, Northampton, Pen Argyl, Bangor, and Broadheadsville.
While prior experience in home care as a Speech Language Pathologist is advantageous, it is not a strict requirement.
As a home care Speech Language Pathologist, SLP, you will play a vital role in a multi-disciplinary healthcare team dedicated to delivering rehabilitative care and skilled nursing, ensuring our clients receive the medical support they need to thrive at home.
Key Responsibilities of the Speech Language Pathologist, SLP:
- Conduct home visits to clients within assigned geographic areas.
- Perform comprehensive evaluations to assess, diagnose, and implement therapeutic interventions for clients facing speech, language, communication, or swallowing challenges.
- Create customized treatment plans based on evaluations, physician directives, and recommended objectives.
- Regularly assess and adjust the speech therapy care plan, collaborating with BAYADA clinicians as necessary.
- Instruct and educate patients, family members, or other representatives on rehabilitation care and activities that support health, safety, and independent living.
- Maintain accurate documentation of observations, interventions, and evaluations related to patient care, utilizing advanced technology such as a touch pad tablet.
