- Assist individuals by offering comprehensive information regarding the recovery process, effective symptom management, harm reduction strategies, and guidance towards self-determination and rehabilitation.
- Conduct outreach to clients to encourage their engagement with services and help them navigate barriers to treatment participation.
- Coordinate necessary supportive services to facilitate the achievement of defined goals through collaboration with housing navigators, case managers, and external service providers.
- Provide daily support to designated participants while actively monitoring program utilization and effectiveness.
- Guide participants in accessing community resources, including housing, transportation, education, and employment opportunities.
- Uphold the highest standards of professional conduct regarding sensitive and confidential information, sharing details only when necessary and in the best interest of those served.
- Comply with and engage in the company’s Health Insurance Portability and Accountability Act (HIPAA) privacy policies and Business Ethics and Compliance protocols.
- Participate in internal staff development initiatives and pursue continuing education as required by company policies and program specifications.
- Continuously develop and refine clinical practice skills.
- Perform additional duties as assigned.
