Your Opportunity
We are seeking a dedicated Therapy Continuity Coordinator to join our team at Dreem Health. In this pivotal role, you will be instrumental in ensuring ongoing patient engagement in therapy long after treatment initiation. This is not a passive follow-up position; you will actively facilitate the continuity journey for patients enrolled in our internal PAP treatment program.
This high-impact role reports directly to the Customer Success Manager, where your contributions will directly influence the stability and growth of our recurring revenue model. You will keep patients engaged through timely outreach, proactive retention strategies, and compassionate communication. Your efforts will aid in reducing churn, establishing a reliable reorder cadence, and providing a seamless resupply experience.
